Policies and Forms

Before your first appointment read the practice policies (4), make copies if you would like them for your records, and complete and return all forms (6).

All forms must be completed 36 hours before your appointment, so Dr. Paras can review them.

Dr. Paras will confidentially send you any additional forms after your first appointment, such as a release of information, or good faith estimate.
