Policies and Forms
After you have spoken to Dr. Paras and set up a first appointment, please read
the practice policies, and make copies if you would like them for your records.
Then complete the initial forms, and be as specific and detailed as you can. This
will save time during our appointment.
All forms must be completed 36 hours before your appointment, so Dr. Paras can
review them. Please initial and sign wherever requested, and have one emergency
contact witness the first page of the registration form.
Dr. Paras will confidentially send you additional forms to your email,
after your first appointment, such as a release of information, the financial
agreement form and the treatment consent form.
